• Report Card for NEW students (original copy and 2 pcs. duplicate copy)
• Certificate of Transfer Credentials, Transcript of Records For TRANSFEREES & GRADUATE SCHOOL STUDENTS (original copy and 2 pcs. duplicate copy)
• Good Moral Character (original copy and 2 pcs. duplicate copy)
• Birth Certificate and  Marriage Contract ( PSA )  if married (original copy and 2 pcs. duplicate copy)
• Classification Test Result / Admission Test Result
• 2×2 latest colored picture 

Additional Requirements for Master’s Degree Program to be conducted by and submitted to Graduate School Dean’s office.

• Certificate of Employment
• Certificate of Weighted Percentage Average (WPA)
• Permit to Study
